About the scale. A BASDAI of 4.0 or higher is the most widely used and best-established cutoff, taken to suggest active disease that may need a treatment review (Garrett et al, 1994; used internationally, including in NICE criteria for biologic eligibility). Beyond this single cutoff there is no universally agreed, formally validated set of BASDAI severity bands, unlike the related ASDAS score, which does have ASAS-endorsed disease-activity categories. Treat any single score as one data point for a conversation with your team, not a diagnosis or a treatment decision on its own.

How the score is calculated

BASDAI = (Q1 + Q2 + Q3 + Q4 + the average of Q5 and Q6) ÷ 5. The two morning-stiffness questions are averaged together first because they measure the same symptom (severity and duration), then combined with the other four.
